That means identifying the disease causing abnormality, showing the therapy targets the root cause or a proximate pathway, leaning on well characterized natural history, and confirming target engagement, with clinical outcomes or validated biomarkers used to support approval when possible.

If that sounds like a loophole, it is better understood as a trade. The trade is earlier access for families who have no time, in exchange for moving more of the uncertainty into careful follow up and post approval evidence. Supporters see this as regulation catching up to modern biology, where sequencing and programmable therapeutics make true one patient style treatments feasible. Skeptics worry that small numbers can hide rare safety problems and can make early success look larger than it is, which is why the details of follow up and quality standards matter as much as the initial decision.

There is also a second, less obvious implication. The FDA explicitly notes that genome editing platforms are highly specific to DNA sequence, and suggests that multiple variants targeting different mutations in the same gene could potentially live under a single application and be studied under master protocols. If that idea becomes real in practice, the long term winner may be the platform, not the one off therapy, because each additional variant can reuse parts of what was already proven about manufacturing, delivery, and mechanism.

This is not a promise that every bespoke therapy will get a green light. It is the FDA signaling that it is willing to evaluate ultra rare disease treatments with a framework built for the world we actually live in, where some patient populations are measured in single digits and the biology can still be unambiguous. The next chapter will be written by how strict the agency is about mechanism, how hard it is to meet manufacturing quality for tiny batches, and how robust the post approval data collection really is.
